USAL conducts a study which shows that 87.5% of patients upon being discharged from hospital have some form of discrepancy regarding their medication

The pilot study of the “Concilia Medicamentos” program developed by the General Council of Pharmaceutical Colleges in collaboration with USAL and CINFA was presented at the 10th Pharmaceutical Congress of Castile and Leon.

Patient safety is a fundamental objective of quality healthcare policies. One of the most common safety problems is the lack of accurate and complete information on the medication taken by the patient. Likewise, there is a high probability during hospital stays that the patient’s pharmacological treatment may be modified. Therefore, coordination among pharmaceutical professionals within the different levels of care is fundamental for avoiding oversights, duplications or interactions, among other types of mistakes. The transition of a patient between levels of care is one of the most common causes for errors associated with drug taking to occur.

This was the objective of the research project “Pilot Study on the Service of Medication Reconciliation of Hospital Patients in the Community Pharmacy” of the “Concilia Medicamentos” program, whose results were presented at the 10th Pharmaceutical Congress of Castile and Leon held on the 25th and 26th of May in Salamanca, specifically at the Hospedería Fonseca of the USAL.
